Faculty Advisor's Certification of New Membership Form

Faculty Membership Application(s) and supporting documentation

Honorary Membership Application(s) (if applicable) and supporting documentation


Mail one college/university check, chapter check, cashier's check or money order covering all fees and dues for all applicants.

Charter fee - $100

Annual dues - $50

Membership dues - $40 per applicant (student, alumni, faculty, & honorary)

The check must be made payable to NOHS-TUA and sent to:

Once the applications are approved, the National Office will send membership certificates and lapel pins to the Faculty Advisor. The chapter may not induct members until their applications are formally approved and the certificates and pins have been received.
