Collection of Applications

1.  Student/Alumni Applications. 

a.  The completed Student or Alumni Membership Applications should be returned to the Faculty Advisor by the due date referenced at the orientation session with all supporting documentation and the lifetime membership fee of $40 paid by the applicant.

b.  The Faculty Advisor must sign each student or alumni application in order to certify that the qualifications for membership have been met. All original documentation must be maintained by the chapter for a minimum of five years. Random audits will be made by the National Office each year to ensure compliance with the Society's by-laws.

c. The Faculty Advisor should complete the Faculty Advisor's Certification of New Membership Form with all the proposed student and alumni members' information. This form must be signed by the Faculty Advisor verifying all members' eligibility and attesting to the fact that each named member has met the minimum standards for membership. After the chapter has been established, the completed and signed Faculty Advisor's Certification of New Membership Form is the only document that must be sent to the National Office for processing.

2.  Faculty Applications.

a.  The completed Faculty Membership Application Form should be returned to the Faculty Advisor by the due date set by the chapter with all supporting documentation and the lifetime application fee of $40 paid by the applicant.

b.  The application must be signed by the Faculty Advisor and sent to the National Office along with all supporting documentation. A copy should be retained by the chapter.

3.  Honorary Applicants.

a.  The Faculty Advisor or designated chapter officer should work with the honorary candidate to ensure the completion of the Honorary Membership Application Form and to make sure that all supporting documentation is collected in time to be submitted with all other applications. The lifetime membership fee of $40 must be paid by the chapter.

b.  The application(s) must be signed by the Faculty Advisor and sent to the National Office with all supporting documentation. A copy should be maintained by the chapter.

Your institution must have a minimum of ten (10) qualified students, alumni, and/or faculty for active membership.
